    *You must complete and return a New 2021/2022 Alternative Income Form before September 16, 2021*

    Our goal is to ensure that RSD students receive the maximum amount of funds that we are eligible to receive by encouraging our families to complete the Alternative Income Form. By taking the time to fill out this form you are taking part in a process that provides thousands of dollars in funds to the school your student attends, including grants, academic coaches, special programs, technology, and more! Only one Alternative Income Form needs to be filled out per household. This is not an application for free or reduced-price meals.

    IMPORTANT NOTES: The submission of this form has no impact on receiving school meals. Not submitting this form may prevent you from receiving a fee waiver or getting access to certain income-based programs. Additional information may be required at the discretion of the school.

    All paper forms are located at every school’s main office. Completed forms can be turned in at any school front office, school cafeteria, or to our Nutrition Services Office located at 1680 Illinois Ave, Unit# 7, Perris, CA 92571.

    *Student(s) enrolled in a Non – CEP schools, Mesa View Elementary, Boulder Ridge Elementary, and Ethan A. Chase Middle School must submit an Alternate Income Form.

    The Community Eligibility Provision (CEP) is a provision from the Healthy, Hunger-Free Kids Act of 2010 that allows schools with a high percentage of students that receive state assistance to provide free breakfast and lunch to all students.

    *Families with students attending Ethan A. Chase Middle School MUST submit an Alternate Income Form for their students or by applying at www.ezmealapp.com
